The YWCA’s Diabetes Wellness & Prevention Program (DWP) Commitments/Details:
– Open to anyone who is Type 2 Diabetic or at risk for Type 2 Diabetes
– Participants commit to a 7-week program with a 6-month follow up for support and accountability to meet your goals.

Requirements include:
– Attend 1 Group Session per week
– Attend 1 Group Fitness Class per week
– Exercise a minimum of 6x per month (with a goal of 2.5 hours per week)
– Maintain Fitness Club Membership

Participants receive $50 for completing the 7-week DSMP program
6-month follow up of weekly support group meetings using the Prevent T2 curriculum (PT2)
1 hour once a week
Participants receive $150 for completing the program

Dates and Times:   6 weeks of the Diabetes Self Management Program (DSMP) plus an introduction and orientation session (7 weeks total)
2 ½ hours once a week
