Description:   Learn how to facilitate Stanford University’s Chronic Disease Self-Management Program (CDSMP) workshops. This FREE 4-day leader training provides you with everything you need to facilitate the CDSMP’s evidence-based 6-session workshop for individuals with chronic health conditions to include diabetes, arthritis, fibromyalgia, asthma, COPD, heart disease, stroke, hypertension, kidney disease, cancer, osteoporosis, depression, and other such chronic health conditions. As Leaders, you will be able to help participants learn how to apply self-management skills to reduce their chronic condition symptoms and associated problems such as tiredness, pain, feeling sad or anxious, and disruptions in daily routines, time with family and friends, and community engagements (church, volunteering, work, and/or educational activities).
