Diabetes Age Well Services
Description: The Diabetes Age Well Services includes six sessions of evidence-based diabetes group classes (Living Healthy with Diabetes) and an individualization assessment and education plan co-created with a Registered Dietitian. Workshops are highly interactive, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. The curriculum was developed by Stanford University. A physician’s referral is required to participate as these are Part B Medicare benefits.
Our program is accredited by the American Association for Diabetes Educators and follows the 2017 National Standards for Diabetes Self-Management Education and Support.
*This class is being offered as a Medicare benefit and is overseen by a registered dietitian. Class times listed below are for the group workshop and additional meeting times will be scheduled one-on-one with a registered dietitian.
